Mentions: The study selection flow chart is illustrated in Figure 1. The electronic database search generated 14,869 papers. After title and abstract reviewing, 206 papers were included for full-text retrieval. Journal hand searching produced 23 more full-text papers. Phase 2 review of the full texts led to the exclusion of 47 additional studies (reasons for exclusion are depicted in Table 1), resulting in 182 papers included for the final analysis. Included studies are listed in the Appendix. There was good inter-reviewer (inclusion/exclusion κ = 0.91; data extraction κ = 0.82) and intra-reviewer agreement (κ = 0.95) during all selection phases.
