Dura-arachnoid Pachymeningeal Enhancement. Schematic diagram. Because the normal thin arachnoid membrane is attached to the inner-layer of the dura, this pachymeningeal pattern is also described as “dura-arachnoid” enhancement.  Enhancement of the dura-arachnoid will be adjacent to the inner table of the skull, in the falx within the interhemispheric fissure; and, also in the tentorium between the cerebellum, vermis, and occipital lobes.  Pure dural (pachymeningeal) enhancement, without pial or leptomeningeal (subarachnoid) involvement, will not fill-in the sulci nor the basilar cisterns.
