Ddx: • Normal petrous fatty bone marrow • Cholesterol granuloma • Endolymphatic sac tumor

Dxhow: Endoscopic surgery

Exam: Physical examination demonstrated right-sided sixth nerve palsy. The remainder of the exam was within normal limits.
