ABSTRACT

Diagnosis: Giant intracranial aneurysm

History: 40 year old woman presents to the Eye Clinic with a new onset of "double vision"

Findings: • 5 cm right Cavernous Internal Carotid Artery Aneurysm • 1.5 cm left Cavernous Internal Carotid Artery Aneurysm

Ddx: • Meningioma • Schwannoma • Pituitary Adenoma • Cavernous Carotid Aneurysm • Carotid-Cavernous Fistula

Dxhow: MRI

Exam: Right Sixth Cranial Nerve Palsy (Abducen palsy)

There is a giant (>25mm) aneurysm (A) in the right cavernous sinus.  There is another, smaller ICA aneurysm (*) in the left cavernous sinus.  The eyes show dysconjugate gaze (arrows), with the right eye unable to move laterally - an abducens (6th) nerve palsy.
