ABSTRACT

Diagnosis: Odontogenic Keratocyst of the Mandible

History: History provided by requesting dentist: Pt presents with radiolucent lesion involving left mandible; please evaluate.

Findings: Extensive involvement of nearly the entire left mandible by a multiloculated/multiseptated cystic lesion. There is no definite cortical disruption or soft tissue component, but there are areas of marked cortical thinning and scalloping.

Ddx: Odontogenic keratocyst Ameloblastoma Aneurysmal bone cyst Less likely fibrous dysplasia due to absence of ground glass appearance

Dxhow: Biopsy-Proven

Exam: Noncontributory

Selected axial and coronal noncontrast CT images of the face show a large multiloculated cystic lesion (confirmed by measurement of Hounsefield Units) involving the majority of the left mandible.  Although there are areas of marked cortical scalloping and thining, there is no definite cortical disruption.  Fluid-levels are not appreciated by CT.
