Suprasellar teratoma with foci of germinoma

ABSTRACT

Diagnosis: Suprasellar teratoma with foci of germinoma

History: This 36 year old man developed emotional lability, constant thirst and temperature intolerance. Endocrine studies were consistent with panhypopituitarism for which hormonal replacement therapy was initiated. Subsequent CT scans disclosed a partially cystic and focally mineralized suprasellar mass lesion with asymmetric hydrocephalus. Craniotomy was performed for resection of the lesion.

Findings: Fig 1. Axial CT showing partially cystic, focally mineralized suprasellar lesion. Note also hydrocephalus with enlargement of the left lateral ventricle. Fig 2. Histological section showing fibrous connective tissue and mature, non-keratinized squamous epithelium. Fig 3. Histological section showing fibrous tissue, adipose tissue and mature hyaline cartilage. Fig 4. Histological section showing mature mucus-secreting columnar epithelium consistent with lower gastrointestinal tract. Fig 5. Histological section showing foci of large polygonal cells with vacuolated cytoplasm and large spherical to ovoid nuclei consistent with germinoma cells.
