ABSTRACT

Diagnosis: Arteriovenous Malformation (AVM)

History: 26 year-old man with increasing right-sided parietal and occipital headaches

Findings: • Right occipital lesion • Serpentine hypointensities with surrounding edema • Enlarged feeding vessels from both PCA and MCA supply • Small feeder from the ACA • Enlarged draining veins - varix -drains into superior sagittal sinus

Ddx: • Arteriovenous Malformation • Aneurysm • Spontaneous intracerebral hemorrhage • Stroke • Intracranial neoplasm

Dxhow: MRI/MRA

Exam: Not available

There is high-flow remodeling of the right anterior circulation and right PCA consistent with arterial venous shunting.  Posterior division of the MCA is enlarged, as is the PCA.
