ABSTRACT

Diagnosis: Adenoid cystic carcinoma arising from lacrimal gland

History: This 72 year old woman developed proptosis and sensory changes over her left eye.

Findings: Fig 1. Contrast enhanced MRI obtained one day after initial biopsy of the lateral intraorbital mass. Fig 2. Gross exenteration specimen with firm gray white neoplasm along the lateral portion of the specimen. Fig 3. Histological section showing glandular diferentiation of the neoplastic cells. The lumina contain mucopolysaccharide secretions. Fig 4. Higher magnification showing relatively uniform neoplastic cells. Mitoses are very rare.

Ddx: Extraconal Mass <li>Lymphoma <li>Lacrimal gland tumor <li>Dermoid/Epidermoid Conal Mass <li>Metastasis <li>Thyroid ophthalmopathy
