ABSTRACT

Diagnosis: Pseudotumor cerebri

History: 39 y/o woman with worsening headaches and blurry vision x 6 weeks.

Findings: • Increased fluid within the optic nerve sheaths • Partially empty sella • Slight flattening of the posterior aspect of the left globe at the optic nerve insertion

Ddx: • Idiopathic intracranial hypertension • Obstructive hydrocephalus • Intracranial mass lesion

Dxhow: Lumbar puncture

Exam: Obese woman VA:20/125 right eye, 20/125 left eye Ishihara color plate 3/11 right eye, 3/11 left eye Extraocular movements intact No Relative afferent pupillary defect Humphrey visual field (30-2): constricted to approx 10 degree OU Moderate optic disk edema 360 degrees OU
