ABSTRACT

Diagnosis: 1. Fractures of the distal second and third metatarsals. 2. Fracture of the medial, distal medial cuneiform. 3. Divergent Lisfranc dislocation.

History: 42 y/o WF presented to the ER complaining of pain and swelling after a horse stepped on her left foot.

Findings: An AP x-ray of the left foot showed fractures of the distal second and third metatarsals. The distal medial cuneiform had a fracture on the medial aspect. The base of the first metatarsal was displaced medially and the base of the second metatarsal was displaced laterally.

Ddx: N/A

Exam: On physical exam the left foot was diffusely swollen and tender to palpation. Bearing weight on the left foot was extremely painful. No labs were ordered.
