ABSTRACT

Diagnosis: Acute Stroke, Hemorrhage in Basal Ganglia

History: 36 year old male with long standing history of poorly controlled hypertension. Patient reported to medical attention with right upper and lower extremity weakness and expressive aphasia.

Findings: Hemorrhage of the putamen with local edema and mass effect.

Ddx: Hypertensive hemorrhage of the putamen Bleeding mass lesion Bleeding vascular malformation

Dxhow: By imaging

Exam: Right upper and lower extremity weakness, expressive aphasia. Blood pressure 200/160.

Axial CT performed with intravenous contrast demonstrates focal high density in the region of the left basal ganglia. There is surrounding low density in the adjacent white matter, and evidence of mass-effect, with partial effacement of the left lateral ventricle.
