History: Patient presented with progressive worsening of headaches with no other signs or symptoms.

Findings: Large pineal region mass.

Ddx: Germinoma Pineocytoma Pineoblastoma Meningioma Metastasis

Dxhow: Pathology, final pathology unknown

In this angiogram, look for what is missing.  Do you see internal cerebral veins?  The vein of Galen?  The straight sinus? There are no dilated veins as the mass effect on the internal cerebral veins has been compensated by the patient's peripheral cerebral venous system.
