ABSTRACT
Diagnosis: Adenocarcinoma History: This is a case of an 82 year old man presenting with acute onset lower gastrointestinal bleeding. Findings: The tagged red cell scan showed activity in the left transverse colon and splenic flexure moving both proximally and distally with time. Arteriography demonstrated an abnormal vascular blush in the arterial recta downstream from the ascending branch of the left colic artery and extravasation of contrast material into the bowel. Following embolization with microcoils, extravasation was no longer appreciated and the study was concluded. A CT study with bowel contrast only demonstrates an apple core filling defect adjacent to the microcoils. Ddx: Ddx of Lower Gastrointestinal Bleed: -Diverticulosis -Hemorrhoids -Neoplasm -Angiodysplasia/AVM -Vasculitis -Ischemic bowel Dxhow: Pathology Exam: Patient had bright red blood per rectum INR-1.2 Cr-1.2 BUN-17 Hgb-13 HCT-30 No MeSH data available. |
