ABSTRACT

Diagnosis: Primary sclerosing cholangitis

History: recurrent pruritis, hyperbilirubinemia and jaundice

Findings: ERCP spot radiograph demonstrates dilatation and stricturing of portions of the extrahepatic and intrahepatic biliary tree. MRCP similarly demonstrates irregularity of the biliary tree. The intrahepatic involvement is primarily limited to the left lobe.

Ddx: Primary sclerosing cholangitis Ascending cholangitis Cholangiocarcinoma Primary biliary cirrhosis

Dxhow: Tissue biopsy by ERCP pathologically confirmed primary sclerosing cholangitis

Exam: jaundice elevated bilirubin
